The following documents are needed be set up with our company:

1. Completed Carrier Contract

2. Copy of your Contract Carrier Authority
   
   
3. Liability Insurance and Commercial General Liability Certificate in the amount of $1,000,000.00 issued by an acceptable insurance company, listing Optimum Source as a Certificate holder.
   
   
4. Cargo Insurance Certificate, with a minimum coverage amount of $100,000 (increased if needed) issued by an acceptable insurance company, listing Optimum Source as a Certificate Holder.
   
   
5. Federal ID number and W-9 form

Please note that the following must be completed for Optimum Source, Inc.
to process your payments of carrier invoices:

1. A signed copy of our Rate Confirmation Sheet
   
   
2. The signed original copy of the shipment Bill of Lading
   
   
3. Your invoice showing OSI load #.
